Patient Portal, Prescription Refills & Medical Records

CHC provides an official patient portal for supported patient-account activities, including medication refill requests and access to selected records or results.

Prescription Refill Steps

  1. Open the official CHC patient resources.
  2. Follow the Patient Portal link.
  3. Sign in using your established patient account.
  4. Select the available medication or refill request feature.
  5. Provide the medication and pharmacy information requested.
  6. Submit the request and allow the care team time to review it.
Medication safety: Submitting a refill request does not mean the medication has been approved or is ready. For urgent medication concerns, contact your CHC provider or pharmacy rather than waiting for a routine portal response.

If you need a medical-record transfer, ask your care team for the correct authorization form. Avoid sending medical records through an unrelated website or unsecured public contact form.
