Not every service operates at every location. PCHC lists its primary medical services in Appleton and dental services at its three dental clinics.
For comparison, our Community Health Center Fort Dodge guide explains how medical, dental and outpatient walk-in services may operate on separate schedules at another federally qualified health center.
How to Book an Appointment
Decide whether you need primary care, a child’s checkup, dental treatment, behavioral health or help applying for insurance.
Dial 920-731-7445. Tell staff whether you are a new or returning patient, what service you need and which clinic is most convenient.
Tell the scheduler whether you have BadgerCare Plus, other insurance or no insurance. Ask about the sliding-fee program if cost may be a barrier.
Ask the scheduler to repeat the clinic name, address, appointment date, arrival time and any instructions about fasting, records, referral paperwork or medications.
Use the official PCHC patient forms page to find the patient information packet, minor consent forms and medical-record authorization forms in available languages.
Bring your identification if available, insurance information, medication list and requested medical or dental records. Ask about financial assistance before treatment if you need a cost estimate.
The general website contact form should not be used for urgent symptoms or appointment cancellations. Call the clinic directly for those requests.
Important 2026 Dental Insurance Change
PCHC says dental care remains available to patients who have:
- BadgerCare Plus / Wisconsin Medicaid coverage;
- BadgerCare Plus / Medicaid in addition to a commercial dental plan or Medicare; or
- No dental insurance and qualify for its sliding fee scale.
This is a dental-department policy. Do not assume it describes every medical insurance contract at PCHC. Ask the scheduling staff to verify acceptance of your specific coverage before your appointment.
Dental cancellation rule: Give at least four hours’ notice when cancelling. PCHC states that shorter notice may count as a missed appointment and affect future scheduling privileges.
Dental treatment-area policy: One support person is permitted in the treatment area under the published guest policy.
Sliding Fee Discounts & Paying Without Insurance
PCHC offers a Sliding Fee Discount Program that may reduce charges based on household income and family size. Insured and uninsured patients are encouraged to apply.
It is not automatically free care. The discounted amount depends on eligibility, the service received and the current fee schedule.
Documents to Prepare
- Photo identification
- Household size and member information
- One month of consecutive paystubs for working household members, if applicable
- Last year’s tax return, if applicable
- Social Security, unemployment or child-support benefit statements, if applicable
- Insurance or Medicaid card, if you have coverage
The discount applies to eligible services performed at PCHC. Certain procedures, devices or dental laboratory charges may not qualify.
Under its Good Faith Estimate policy, eligible uninsured or self-pay patients may request written expected charges. PCHC explains that patients can dispute qualifying bills that exceed the estimate by more than $400.

BadgerCare Plus, Marketplace & Insurance Help
The PCHC Outreach and Enrollment team assists people who need help understanding or applying for healthcare coverage and community benefits.

| FoodShare | Wisconsin FoodShare application assistance. |
| Financial hardship | Sliding-fee eligibility and available financial assistance. |
| Other community needs | Referrals for food, housing, transportation and other local support. |
Outreach office: 119 N. McCarthy Road, Suite S, Appleton, WI 54913, door #21.

Direct phone: 920-882-6420.
Behavioral Health & Recovery Support
PCHC’s behavioral health providers work with primary care teams to address emotional wellbeing, stress, mental health symptoms and substance-use concerns. Services may include counseling, clinical assessment, medication-related support and referrals when appropriate.
The official program describes support for depression, anxiety, ADHD, grief, trauma, relationship difficulties, anger management and behavior change. It also offers EMDR therapy for selected patients when clinically appropriate.
For recovery from opioid or other substance-use disorders, ask about the Medication Assisted Recovery program and the available assessment process.
For a broader example of behavioral healthcare combined with affordable community health services, our Community Health Center Everett patient guide covers medical, dental and behavioral health department arrangements.
