How to Book an Appointment at Apicha

Patients can request an appointment using Apicha’s official online request form or by telephone. Completing an inquiry form does not mean a date has been confirmed.

  1. Choose the clinic. Select Manhattan or Queens based on the department you need.
  2. Identify your appointment type. Primary care, pediatrics, dental, mental health or another service.
  3. Contact the center. Call 212-334-6029 for Manhattan or 718-567-5200 for Queens. New patients can also ask for help at 212-334-7940.
  4. Use the appointment form if preferred. On the official appointment page, select your appointment type and follow the displayed steps.
  5. Enter requested details. The form may ask for your name, contact details, ZIP code, date of birth and appointment category.
  6. Await scheduling confirmation. Confirm your assigned time, department, location and whether you need any referral or special preparation.
  7. Bring required documents. See the new-patient checklist below and arrive according to the clinic’s instructions.

What to Bring to Your Visit

  • Photo identification.
  • Medical, dental and pharmacy insurance cards, if applicable.
  • Copayment, if required by your coverage.
  • Proof of income if applying for sliding-fee assistance.
  • Current medication list and important medical records.
  • Referral information if your appointment requires one.
  • Questions you want to discuss with your provider.

The first four requirements above are specifically identified in Apicha’s published patient handbook. Ask staff what to do if you do not have a particular document.

Changing an appointment: Apicha’s patient handbook says patients can cancel using the SMS reminder sent three days before the appointment. For other changes, call the clinic promptly.

Insurance, Sliding-Scale Discounts & Costs

Apicha states that it does not deny healthcare because a person cannot pay. Its sliding-fee discount program is available to qualifying insured and uninsured patients.

Sliding-Fee Eligibility

Discounts are assessed based on household income and family size. The program may help eligible patients manage the cost of care, including those who already have health insurance.

Exact prices and discount amounts depend on your assessment and service.

Insurance Enrollment

Apicha offers assistance with the New York State of Health Marketplace, insurance applications and renewals.

Ask enrollment staff which plans the clinic currently accepts and whether your chosen provider is in network.

Documents for Sliding-Scale Screening

Apicha identifies the following examples of acceptable income documentation:

  • Recent pay stub from within the previous 60 days.
  • W-2 or unemployment statement.
  • Disability, Social Security or public assistance documentation.
  • Employer letter or household support letter.
  • Self-declaration, when accepted under the program.

Family size can include the patient, spouse or partner, supported children and other individuals included on the patient’s federal income taxes, as described in its handbook.

How to Request a Discount

  1. Tell the patient associate or case manager you want to be screened for sliding-fee assistance.
  2. Explain your insurance and household situation.
  3. Provide the appropriate proof of income and family-size information.
  4. Ask for the estimated patient charge before receiving a non-urgent service.
  5. Check whether follow-up visits, medication and specialty services are billed separately.
Cost tip: Do not assume that uninsured care is automatically free, or that being insured prevents you from qualifying for a discount. Request an individual eligibility assessment.

HIV, STI Testing & Sexual Healthcare

Apicha began as an HIV-focused organization and continues to provide HIV prevention and treatment, sexual healthcare and related patient support.

Testing & Treatment

  • HIV testing and linkage to medical care
  • STI evaluation and treatment
  • Sexual health counseling
  • Prevention services and education
  • Ongoing HIV-related healthcare

Prevention Options

  • PrEP evaluation and prescriptions
  • Doxy-PEP counseling when appropriate
  • Long-acting HIV prevention options when clinically available
  • Sexual-health risk-reduction guidance
  • Care coordination and follow-up

Free & Confidential Testing Contact

Apicha lists 866-274-2429 for free and confidential HIV and STD testing inquiries.

Call to confirm test types, free-testing eligibility, available days, whether an appointment is required and when results can be expected.

Time-sensitive HIV exposure: If you may have had a recent HIV exposure, seek urgent medical evaluation immediately. HIV post-exposure prophylaxis (PEP) must generally begin within 72 hours and works best when started sooner. Do not wait for a routine clinic appointment if it would delay treatment.

Patient Portal, Referrals & Health Records

Patient Portal

Apicha provides a patient portal resource for its patients. Access instructions and account help are available from the clinic.

  1. Open Apicha’s patient portal information page.
  2. Follow the official access or enrollment instructions.
  3. Use your verified patient credentials.
  4. Contact the clinic if you need account activation or help signing in.

Specialist Referrals

Apicha’s handbook explains that your primary care provider can refer you to a specialist or imaging service.

For an insurance plan requiring an electronic referral, notify your care team at least 72 hours before a specialist appointment, with the specialist’s name, NPI and phone number.

Ask the referral team whether authorization is complete before attending.

Available In-House Tests

The patient handbook lists limited in-house testing that may include HIV, urinalysis, urine pregnancy, glucose, mpox, COVID-19, flu, strep and RSV. Specific testing depends on clinical need, supply and current service availability.

Call before visiting solely for a test, especially if you need a specific result deadline.

Language Access & Culturally Responsive Care

Apicha has served New Yorkers since 1989 and has particular experience serving AAPI communities, LGBTQ+ patients, people living with HIV and people with limited English proficiency.

Its provider directory includes language filters such as English, Spanish, Cantonese, Bangla, Tagalog, Russian, French, Ukrainian and Bahasa Indonesia. Not every provider speaks every language.

How to Find a Language-Matched Provider

  1. Visit the official provider directory.
  2. Choose Manhattan or Queens.
  3. Select the medical specialty.
  4. Use the language filter where available.
  5. Call the clinic to confirm appointment availability or interpretation arrangements.
